On the 8/22/12 designated operator monthly report it was stated that 91 STP sump had 25 gallons <br /> of fuel and the alarm was on from 8/17/12. The leak was not repaired and reported to the EHD within 24 <br /> hours and a full report was not submitted to the EHD within five working days. Any release from the <br /> primary containment that does not escape the secondary containment and was not cleaned up within <br /> eight hours of detection, or any release that escapes the secondary containment shall be reported to the <br /> EHD within 24 hours and a full report shall be submitted within five working days. The report shall <br /> include at least the following information: <br /> 1. Owner's or operator's name and telephone number <br /> 2. List of types, quantities, and concentrations of hazardous substances released <br /> 3. Approximate date of release <br /> 4. Date release was discovered <br /> 5. Date release was stopped <br /> 6. Description of actions taken to control and stop the release <br /> 7. Description of corrective and remedial actions <br /> 8. Methods of clean up implemented to date, and proposed clean up actions with a time schedule <br /> 9. Method and location of disposal of waste generated from release and clean up. Attach copies of <br /> any hazardous waste manifests. <br /> 10. Description of proposed methods of repair or replacement of primary or secondary containment <br /> 11. Description of additional actions taken to prevent future releases. <br /> Immediately prepare a report with the required information and submit a copy to the EHD within 30 days <br /> of receiving this report. Ensure that any future releases are reported appropriately. <br /> 302. The 91 STP sump sensor was in alarm from 8/17/12 but was ignored by the owner (Designated <br /> Operator found 25 gallons of fuel in the sump 5 days later). The monitoring system is not working <br /> properly to ensure continuous monitoring of the tank system. Immediately, alarms should be <br /> acknowledged and appropriate corrective action taken. Failure to correct alarms in a timely manner <br /> could lead to severe health and environmental problems.
