Failure to submit the completed information immediately may <br /> result in a re-inspection, additional violations, and/or further legal action. <br /> Return to compliance information was required to be submitted to the EHD by February 9, 2014; to date, <br /> no corrective action information was received by the EHD. <br /> Immediately address all existing violations and provide the following documentation to the EHD: <br /> ✓ Return to Compliance Certification Form (attached hereto) <br /> A statement- describing how each violation was corrected, including supporting <br /> documentation <br /> Violation 105: UST Facility Operating Permit Application and UST Tank Information are not <br /> uplpoaded in CERS. Immediately log into CERS, upload all the required information, and <br /> submit for review by the EHD. <br /> V Violations 109, 110,& 111: Annual monitoring system certification, leak detector testing, and <br /> spill container testing were last performed on December 27, 2012 and was completed on <br /> January 9, 2014 and is considered one month past due. These tests are required once every <br /> 12 months. Take all necessary precautions to ensure testing is performed in a timely <br /> manner. <br /> V Violation 204: The designated operator failed to check and note the correct (most current) <br /> test dates for the monitoring system certification on the January 2013 through April 2013 <br /> designated operator monthly inspection reports. Ensure that designated operators performing <br /> monthly inspections at this facility are including all of the required information on the reports. <br /> 1 <br />
