0 0 <br /> ✓ Violations 302 & 308: Jumper hoses, connecting the flexible product piping, were found <br /> inside all the STP sumps and inside the UDCs 1/2 through 5/6. Immediately contact a <br /> properly licensed, trained, and certified contractor to ensure that any releases in the 87 and <br /> 91 piping flows into the UDC 3/4 sump. <br /> ✓ Violations 108 & 307: The most recent secondary containment test results (December 19- <br /> 20, 2012) indicated a failure at UDC 1/2, UDC 3/4, and UDC 5/6. The secondary <br /> containment testing was performed on March 25, 2013 and a test report has not been <br /> submitted to the EHD. Immediately submit a copy of the test report to the EHD. <br /> Immediately have a properly licensed, trained, and certified contractor repair or replace the <br /> components failed during the secondary containment test under permit and inspection of the <br /> EHD. <br /> Immediately complete and submit a copy of the Return to Compliance Certification form with a statement <br /> of how each violation was corrected, including supporting documentation, to the EHD.
