Financial responsibility documents have not been submitted to the EHD. Current financial <br /> responsibility documents are required to be submitted annually. Updated financial responsibility <br /> documents were received at the time of inspection. CORRECTED. <br /> 109, 110, & 111. Annual monitoring system certification, leak detector testing, and spill container testing <br /> were last performed on December 27, 2012 and was completed on January 9, 2014 and is considered <br /> one month past due. These tests are required once every 12 months. Take all necessary precautions <br /> to ensure testing is performed in a timely manner. <br /> 113. Mr. Lyle Meeks performed several monthly designated operator inspections (February 2013 <br /> through November 2013) and a notification was not provided to the EHD within 30 days of the change. <br /> Any designated operator changes shall be reported to the EHD with 30 days. An updated notification <br /> identifying Lyle was submitted to the EHD at the time of inspection.identifying all the designated <br /> operators for this facility. CORRECTED. <br /> 204. The designated operator failed to check and note the correct (most current) test dates for the <br /> monitoring system certification on the January 2013 through April 2013 designated operator monthly <br /> inspection reports. During the monthly inspection, the designated operator shall check that all required <br /> testing and maintenance for the UST system have been completed and document the dates they were <br /> done.
