Jumper hoses, connecting the flexible product piping, were found inside all the STP sumps <br /> and inside the UDCs 1/2 through 5/6. As such, any potential release from the primary containment into <br /> the secondary containment in the 87-octane and 91-octane flexible piping does not flow into a collection <br /> sump. All secondary containment systems shall be sloped so that all releases will flow to a collection <br /> sump located at the low point of the underground piping. The technician onsite disconnected the jumper <br /> hoses located all STPs sumps and UDCs 1/2 and UDCs 5/6, however, he was unable to disconnect the <br /> hoses in UDC 3/4 due to a deeper jumper hose placement. Immediately contact a properly licensed, <br /> trained, and certified contractor to ensure that any releases in the 87 and 91 piping flows into the UDC <br /> 3/4 sump. <br /> 303. The 87-octane sump sensor was located approximately 3-inches up the sump saddle and not <br /> located to detect a leak at the earliest opportunity. Monitoring equipment shall be maintained to be able <br /> to detect a leak at the earliest possible opportunity. The technician onsite repositioned the sensor to <br /> detect any future leak at the earliest opportunity. CORRECTED. <br /> 305. The UDC 3/4 sensor failed to activate an audible and visual alarm when tested to ensure <br /> continuous monitoring of the tank system. All monitoring equipment shall be maintained to activate an <br /> audible and visual alarm or stop the flow of product at the dispenser when it detects a leak. The <br /> technician onsite cleaned the sensor, retested, and it passed. CORRECTED. <br /> 108 & 307. The most recent secondary containment test results (December 19-20, 2012) indicated a <br /> failure at UDC 3/4 and UDC 5/6. All secondary containment for the UST system must be tight. A permit <br /> was obtained from the EHD on March 15, 2013 (SR0066337) to repair the aforementioned components. <br /> According to an email communication from Franzen-Hill Corporation, dated April 17, 2013, a successful <br /> secondary containment testing was conducted for the UDC 5/6 on March 25, 2013. According to the <br /> aforementioned correspondence, additional repairs are still necessary for the UDC 1/2. Immediately <br /> have a properly licensed, trained, and certified contractor repair or replace the failed components under <br /> permit and inspection of the EHD. Furthermore, according to Franzen-Hill Corporation, a secondary <br /> containment testing was performed on March 25, 2013 and a test report has not been submitted to the <br /> EHD. A copy of the test report must be submitted within 30 days of the test.
