You can ignore pages 4, 5, and 6 of <br />APPLICATION FOR UNDERGROUND STORAGE TANK RETROFIT OR PIPING REPAIR PERMIT, and for <br />contractor you could put "to be determined" for now and provide contractor information as soon as possible. <br />http://www.socehd.com/Programs/Others/Credit Card Payments.htm to ay fees online. <br />Please don't hesitate to call me if you have any questions. <br />Elena K.
