A new designated operator was hired at least a year ago and notification was not provided to the EHD within 30 days of the change. Jacob Weaver, Gabriel Vonegas, Jpse Ochoa, and James Flowers have conducted the designated operator inspections in 2015 and the notification was not provided to the EHD. Any changes shall be reported to the EHD within 30 days.
