On the 11/13/18 designated operator(DO)inspection report under -Section III The DO failed to add a description to all the components marked N/A or N from section VII through XI. -Section VI The OWNER/OPERATOR failed to provide a name, signature, and date within 48 hours of receiving the DO report under the ACKNOWLEDGMENT OF COMPLIANCE ISSUES section. -Section VII The DO fail to mark yes or No for the question"Has each follow-up action of section III from the previous inspection been completed appropriately?' The alarm history reports for the1/2/18, 9/24/18, and 10/26/18 inspection reports were not found on site. The designated operator failed to verify that the alarms from regular sump on 2/2/18,7/26/18, 12/14/18 and 12/15/18 Premium sump on 6/7/18 were responded to appropriately. In the DO report for 7/20/18 the DO marked YES under the question"sensor located in tank top containment sumps have not alarmed in the past month?'however on 7/15/18 there was a regular sump alarm and it should state NO. During the monthly inspection,the designated operator shall review the alarm history for the previous month,check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing inspections at least once every 30 days at this facility and are including all of the required information on the reports.
