(#201. CCR 2712(b) & 109. CCR 2638). Monitoring system certification and spill bucket test results from 11/19/09 were not found on site. These records shall be kept on site for at least 3 years. All these test were last performed by Epic Compliance Systems. Immediately locate the test results and maintain on site. - Chris Arbabian stated the test results could not be located but it will be resolved. Continue contacting the testing company to retrieve a copy for facility's records. #23c. (#205. CCR 2715(e)). February and March 2010 designated operator monthly inspection reports were not found on site. Last designated operator monthly report from Epic Compliance was found for January 2010. Franzen Hill monthly inspection reports began in April 2010. Designated operator monthly inspection reports for the previous twelve months shall be retained on site. Located and ensure that copies of the previous 12 months of designated operator inspection reports are maintained on site. - Chris Arbabian stated he will contact his designated operators for copies of February and March 2010 reports. It is unclear if Chris communicated with his former designated operators for missing reports. Continue contacting the designated operators to retrieve copies of February and March 2010 reports. Submit to SJC EHD within 30 days (04/15/13): 1. one copy of the completed "Return to Compliance Certification" form for 2013 2. one copy of the completed "Return to Compliance Certification" form for 2011 3. a written corrective action statement for each of the 2013 violations 4. a written corrective action statement for each of the 2011 unresolved violations 5. submit online http://cers.calepa.ca.gov facility's HMBP, UST reports. make appoint with me if you are unable to complete the reporting. Notes: -A copy of financial responsibility dated 02/2013 was on site but no copy was available for inspector. A faxed copy was received 02/26/13. Ensure a copy is submitted to EHD annually via fax or postal mail. Last year copy was dated 03/2012. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
