Secondary containment test results from 06/03/08 and Monitoring System Certification, Leak Detector and Spill Bucket test results from 11/09/09 were not found on site. These records shall be kept on site for at least 3 years. All these tests were last performed by Epic Compliance Systems. Immediately locate both test results and maintain the on site. 23c. The designated operator failed to check and note the correct test dates for the all required testing on all monthly visual inspection reports since 04/29/10. Report for April - June 2010 documented monitoring system certification and spill containment/ bucket test date as 10/07/09 and no date "no records" for secondary containment testing. Reports for July - December 2010 documented monitoring system certification and spill containment/ bucket test dates as 07/31/10 and no date "no records" for secondary containment testing. Only a cold start of the monitoring panel was performed on 07/31/10 but this is not a complete annual monitoring certification testing since it did not include leak detector and spill bucket testing. During the monthly inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. 23c. The February and March 2010 designated operator monthly inspection reports were not found on site. Last designated operator monthly report from Epic Compliance was found for January 2010. Franzen Hill monthly inspection reports begin in April 2010. Designated operator monthly inspection reports for the previous twelve months shall be retained on site. Locate and ensure that copies of the previous twelve months of designated operator monthly inspection reports are maintained on site. Submit copies to the EHD by 02/28/11. 24. Test reports were not submitted within 30 days. Last monitoring system certification was completed on 11/19/09. Leak detector and spill container testing were completed on 11/30/09. No test results were received for the monitoring system certification test. Leak detector and spill container test results were received by EHD on 01/28/10 - two months past due. Ensure all future test results are submitted within 30 days to testing to EHD. Submit a copy of the monitoring system certification test results from 11 /19/09 to E H D by 02/28/11. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($122).
