Annual monitoring system certification, leak detector, and spill container testing were last <br />performed on 11 /19/09 and was due by 11 /2010 . An incomplete monitoring system certification today - <br />87 annular was not able to be removed from the interstitial space to be tested since the protective outer <br />sheath covering of the black and red wires was in poor condition and frayed. The wiring was exposed <br />and pulling the annular sensor out would cause tension and possible breakage. The 87 annular sensor <br />is to be reschedule next week. Spill buckets and leak detectors are completed. These tests are required <br />once every 12 months. Immediately schedule this test and provide 48 hours notification to the EHD. <br />Please be aware that a follow up inspection will be charged at our current hourly. Repeat violation. <br />Submit to SJC EHD within 30 days (02/28/11): <br />1. one copy of the completed "Return to Compliance Certification" form <br />2. a written corrective action statement for the above violations <br />3. requested/ supporting documents ie. test results <br />Note: <br />When the annual monitoring system certification, line leak detector and spill bucket/ containment testing <br />is reschedule, the inspection will be a chargeable inspection at $122/ hour. Both owner and testing <br />company clerical assistant are aware of the fee. <br />- Found May & July 2010 DO monthly reports. <br />- Found DO employee training last done in April 2010. <br />- Received copy of Owner Statement of DO for Steve Zwahlen. <br />- Completed UST facility, tank, Financial Responsibility, Monitoring Plan (approved).
