UST facility and tank forms on file with the EHD are not current. Any change of information on these <br />forms must be submitted to the EHD within 30 days of the changes. Immediately complete and submit a <br />copy of the facility and tank forms to the EHD by 02/28/11. Forms can be located at the website found <br />on the header of this document. Complete a separate tank form for each tank. Repeat violation. <br />4. Financial responsibility documents have not been submitted to the EHD. Last financial responsibility <br />document received on 12/18/08. Financial responsibility is valid only for one year. Current financial <br />responsibility documents are required to be submitted annually. Complete and submit a copy of the <br />financial responsibility (Certification of Financial Responsibility and Letter from Chief Financial officer or <br />equivalent) by 02/28/11. Repeat violation. <br />5 & 6. Current approved copies of the Monitoring and Response Plans were not found on site. The <br />Monitoring and Response Plans must be current and approved by the EHD. EHD has on file a copy of <br />the approved Response Plan only. Complete and submit a copy of the Monitoring Plan for approval by <br />the EHD by 02/28/11. An approved copy will be returned for your records. Repeat violation. <br />7. Secondary containment test results from 06/03/08 and Monitoring System Certification, Leak Detector <br />and Spill Bucket test results from 11/09/09 were not found on site. These records shall be kept on site <br />for at least 3 years. All these tests were last performed by Epic Compliance Systems.
