A new designated operator Steve Zwahlen had performed all of facility's monthly visual inspection reports since 04/29/10 and notification was not provided to the EHD within 30 days of the change. Steven Zwahlen had incorrectly documented on the monthly visual inspection report that his designated UST operator certification expires 05/01/12. His certification expires 03/31/12. EHD had received a fax of Owner Statements of Designated UST Operator on 01/20/11 from Franzen -Hill that Tony Murch was the designated operator for this facility and it was signed by tank owner on 04/19/10. Tony Murch has not performed any monthly visual inspections reports for this facility. Any changes shall be reported to the EHD with 30 days. Submit notification to the EHD identifying all the designated operators for this facility by 02/28/11. Repeat violation. 23c. The designated operator failed to check and note the correct test dates for the all required testing on all monthly visual inspection reports since 04/29/10. Report for 04/29/10 and 06/29/10 documented monitoring system certification and spill containment/ bucket test date as 10/07/09 and no date "no records" for secondary containment testing. Reports for 08/31/10, 09/28/10, 10/27/10, 11/29/10, 12/29/10 documented monitoring system certification and spill containment/ bucket test dates as 07/31/10 and no date "no records" for secondary containment testing. During the monthly inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. 23c. The designated operator failed to visually inspect the UST system for leaks, water, or debris during the monthly inspection. No monthly visual inspection reports were found for May and July 2010. During the monthly inspection, the designated operator shall visually inspect all spill containers and UDC sumps, and any tank top sump that had an alarm for which there is no record of a service visit. Ensure that designated operators performing monthly inspections at this facility are inspecting all of the required areas and noting it on the on the reports. 23c. The designated operator monthly inspection report for May and July 2010 were not found on site. Designated operator monthly inspection reports for the previous twelve months shall be retained on site. Locate and ensure that copies of the previous twelve months of designated operator monthly inspection reports are maintained on site. Submit copies to the EHD by 02/28/11. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($122).
