The designated operator employee training records were not found. The designated operator shall train facility employees for which he or she is responsible in the proper operation and maintenance of the UST system once every 12 months. The training shall include, but is not limited to: 1. Operation of the UST system in a manner consistent with the facility's best management practices 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan 3. Employee's role with regard to spills and overfills as specified in the facility's response plan 4. Name of the contact person(s) for emergencies and monitoring equipment alarms Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of the training records to the EHD by 02/28/11. 24. Test reports were not submitted within 30 days. Last monitoring system certification was completed on 11/19/09. Leak detector and spill container testing were completed on 11/30/09. No test results were received for the monitoring system certification test. Leak detector and spill container test results were received by EHD on 01/28/10 - two months past due. Ensure all future test results are submitted within 30 days to testing to EHD. Submit a copy of the monitoring system certification test results from 11/19/09 to EHD by 02/28/11. 24, 25, & 26. Annual monitoring system certification, leak detector, and spill container testing were last performed on 11/19/09 and was due by 11/2010. Testing has not been completed and was schedule to be completed on 01/27/11 but technician was unavailable. It is to be reschedule to 02/07/11 and will be four months past due. These tests are required once every 12 months. Immediately schedule these tests and provide 48 hours notification to the EHD. Please be aware that a follow up inspection will be charged at our current hourly. Repeat violation. Submit to SJC EHD within 30 days (02/28/11): 1. one copy of the completed "Return to Compliance Certification" form 2. a written corrective action statement for the above violations 3. requested/ supporting document. ie. test results Note: When the annual monitoring system certification, line leak detector and spill bucket/ containment testing is reschedule, the inspection will be a chargeable inspection at $122/ hour. Both owner and testing company clerical assistant are aware of the fee. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($122).
