0 0 <br /> d. Each person installing or testing any component must hold a certificate of <br /> training from the manufacturer of that component. <br /> 4. Any changes shall be submitted to SJC Environmental Health Department(EHD) <br /> for approval prior to commencing work. All new submittals supersede the <br /> previous ones. <br /> 5. The following information must be submitted within 30 days of the sample date: <br /> a. Analytical results sent directly from a California certified laboratory or <br /> this office WILL NOT accept them. <br /> b. See page 4 of the permit application for required analytical tests. <br /> 6. All personnel involved in the removal of the existing pipes must fulfill <br /> requirements of CCR, Title 8, Section 5192. <br /> 7. If any fuel is to remain in the underground storage tank, it must be continuously <br /> monitored. <br /> 8. All piping, fittings, sumps and UDC°s must be certified by an independent testing <br /> organization and labeled as required by the organization. (UL, ULC, ANSI,API, <br /> ASTM, etc., See California Code of Regulations, Title 23, Chapter 16 Table B for <br /> complete list.). Pursuant to the Underwriters Laboratories Inc(UL) listings, only <br /> those products bearing the UL mark will be considered listed. Any part/product <br /> not marked with the UL label or another listed testing organization will be <br /> rejected. <br /> 9. All installation and testing shall be in compliance with manufacturer's <br /> instructions, Section 2635 and 2636 of CCR. <br /> 10. All piping must slope and be able to drain to a containment sump with an <br /> audible/visual alarm. <br /> 11. All metal components in direct contact with the ground such as flexible <br /> connectors, swing joints, fittings, and pumps shall be cathodically protected. <br /> 12. Pipeline and tank secondary containment testing must be performed in accordance with <br /> the manufacturer's guidelines. Testing must be done after returning the facility to <br /> operating condition. Results must be received by the EHD within seven days of returning <br /> the facility to operating condition. Piping must be retested six months after repair, and <br /> results submitted to the EHD within 30 days in order for secondary containment <br /> testing to be considered complete for the repaired portion of the system. <br /> 13. A"Certificate of Compliance for Underground Storage Tank Installation Form C" <br /> shall be submitted by the owner or the owner's agent before this installation will <br /> be considered complete by EHD. <br /> 14. Contact Michelle Henry, REHS ay(209) 953-7699 for the following inspections: <br /> A. Piping removal and sampling <br /> B. Pressure test on Primary line <br /> C. Secondary piping soap and pressure tests <br /> D. 1-Hour lake test on sump(s) <br /> E. Visual inspection of piping to insure proper slope prior to <br /> covering <br /> F. Cathodic protection on metal components in direct contact with <br /> backfill material if necessary <br /> This permit expires 180 days from the approval date. A request for extension must be <br /> received 30 days prior to the expiration date. <br />
