Waterloo Rd., Stockton Program: UST <br /> SUMMARY OF VIOLATIONS <br /> CLASS I,CLASS 11,or MINOR-Notice to Comply) <br /> This is a Routine and Monitoring Certification inspection. The following repeat violations were observed <br /> during this inspection: #4, 7(M1), 9, 11(M1), 17(M1), 21, 7 ), 27(M1) <br /> 4. Financial responsibility documents have not been submitted to the EHD. Current financial <br /> responsibility documents are required to be submitted annually. Complete and submit a copy of the <br /> financial responsibility by July 30, 2011. A blank form has been provided to you for this purpose. <br /> 7. Maintenance records were not found on site, and daily alarm log has not been kept since 2010. <br /> These records shall be kept on site for at least 3 years. Immediately locate and maintain the on site <br /> and submit copies to the EHD by July 30, 2011. <br /> 9. Diesel and 91-octane STP alarms are not functioning properly, and line leak detection for the diesel <br /> line could not be verified since the diesel pump could not be activated. The underground storage tank <br /> system shall be operated to prevent unauthorized releases. See Item #17 and 27 below for corrective <br /> action requirement. <br /> 11. The Diesel-grade STP sensor was found not located at the bottom of the tank to detect a leak at the <br /> earliest opportunity. Monitoring equipment shall be maintained to be able to detect a leak at the <br /> earliest possible opportunity. Immediately relocate this sensor to the lowest point of the tank. The <br /> length of the cord was adjusted by technician on site to locate the sensor to the bottom of the tank; <br /> violation was corrected. <br /> 17. The diesel line leak detector was not tested to detect a leak. All line leak detectors shall be capable <br /> of detecting a 3-gallon per hour leak at 10 psi. Immediately have a properly licensed, trained, and <br /> certified contractor repair or replace the failed leak detector (LG 113-x listed, if applicable) and obtain a <br /> permit within one business day from the EHD. If the failed leak detector can't be replaced within 7 days, <br /> there is a possibility that the UST system may be red tagged to prevent fuel inputs. <br /> 21. Liquid was observed in the Dispenser 5/6 UDC sump. If water could enter into the secondary <br /> containment by precipitation or infiltration, it must be removed and disposed of properly. Immediately <br /> remove this liquid, make a hazardous waste determination per Title 22 hazardous waste regulations, <br /> and manage it accordingly. Ensure that the sump is maintained free of liquid. About one gallon of liquid <br /> was collected by technician on site and disposed as hazardous waste drum on site. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EH spector• Received By: Title: <br /> EHD 02-0 3 Rev8 0/10 CONTINUATION FORM <br />
