The Dispenser#3/4 UDC sump sensor failed to shut down the 91-octane pump, the 91-octane STP <br /> sensor failed to shut down the 91-octane pump, and the diesel STP sensor could not be verified to shut <br /> down the diesel pump (but was noted to shutdown the 91-octane pump instead). The diesel and 91- <br /> octane lines do not meet the requirements for annual line tightness testing exemption. Annual line <br /> tightness testing is required unless the UST system is equipped with the following: <br /> 1. A continuous monitoring system that either activates an audible and visual alarm or stops the flow <br /> of product at the dispenser when it detects a leak <br /> 2. A monitoring system that shuts down the pump or stops the flow of product at the dispenser when <br /> it detects a leak <br /> 3. A monitoring system for piping other than that in UDC sumps (tank top sumps, transition vents, <br /> etc.) is failsafe and shuts down pump when a leak is detected <br /> Immediately schedule this test by properly licensed, trained, and certified contractor, and provide 48 <br /> hours notification to the EHD. A permit is required if the system had to be repaired, reprogrammed, or <br /> components replaced. If this could not be verified under EHD inspection within 7 days, additional <br /> enforcement action may be taken. <br /> NOTES <br /> Facility manager stated the diesel dispenser is down; therefore the pump could not be activated / <br /> authorized. <br /> 25. Annual monitoring system certification and leak detector testing was last performed on June 30, <br /> 2010 and was due by June 30, 2011. The sensor-out and fail-safe portion of the monitoring certification, <br /> and diesel line-leak detector could not be performed today. These tests are required once every 12 <br /> months. Immediately schedule these tests and provide 48 hours notification to the EHD. Please be <br /> aware that a follow up inspection will be charged at our current hourly rate. <br /> Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br /> each violation was corrected, including supporting documentation, by July 30, 2011. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS F CILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EH pecto Received By: Title: <br /> EHD23-02-OV RL0811CU CONTINUATION FORM <br />
