The scope of work includes installing <br />Pressure Management Control software and interface module on the VR TLS -350 console which <br />requires a "cold start", and constructing new conduits in the concrete from the dispenser island to <br />the new J -Box at the 87 underground storage tank (UST). This project also includes converting <br />the 89 tank to red diesel by removing product from the UST, haul off by a certified hauler (Uni- <br />Waste) to a permitted facility (Clearwater), and installing new mechanical leak detector VMI 99 - <br />LD -2000. The permit has been approved with the following conditions: <br />1. Contact the local fire department, local planning or building department and the San <br />Joaquin Valley Air Pollution Control District for any applicable permits. <br />2. Any changes shall be submitted to the San Joaquin County Environmental Health <br />Department (EHD) for approval prior to commencing work. All new addenda supercede <br />the previous ones. <br />3. Before the alarm history is cleared by the "cold start", a current printout must be <br />created and available for EHD inspection. Inspection may occur during the monitoring <br />system functionality test. <br />4. Hauler of hazardous waste must possess current Active Transporter Registration from <br />DTSC. <br />5. If concrete is broken over the existing underground storage tanks (UST) All components <br />must be certified by an independent testing organization and labeled as required by the <br />organization. (UL, ULC, ANSI, API, ASTM, etc. See California Code of Regulations <br />(CCR), Title 23 Chapter 16 Table B for complete list). The component must bear the <br />appropriate mark as required by the independent testing organization. <br />6. Qualified service technicians must possess or work under the direct and personal <br />supervision of an individual physically present at the work site who possesses all of the <br />following: <br />a. One of several permissible valid, current contractor's licenses issued by the <br />Contractors State License Board (may be issued to the installation company), and <br />Page 1 of 2 77 <br />
