All valid, current California UST certificates issued by the International Code <br />Council (ICC), that are applicable to the work being performed, and <br />c. A valid, current certificate of training from the manufacturer(s) of the <br />component(s) being tested, repaired, or serviced; and <br />d. Each person installing or testing any component must hold a certificate of <br />training from the manufacturer of that component. <br />7. Contact Jeffrey Wong (468-0335) to arrange the following inspection(s). A 48-hour <br />notice is required: <br />a. The EHD must witness the functionality of the monitoring system to ensure <br />that monitoring has been restored to meet the requirements of California <br />Code of Regulations Title 23 and the Health & Safety Code. All monitoring <br />system sensors must be tested. <br />b. Test and verify that the new VMI leak detector will detect a leak of 3 <br />gallons per hour at 10 PSI for the diesel tank. <br />c. EHD must be present to verify if concrete is broken to cap off the <br />vapor line at the fill sump of the 89 tank. <br />8. A secondary containment test must be performed on both 87 and former 89 tanks, and <br />both product lines from the two tanks, in accordance to manufacturer's guidelines. <br />Testing must be done after returning the facility to operating condition. Results must be <br />submitted to EHD within 7 days of returning the facility to operating condition. <br />9. An "Underground Storage Tank Certification of Installation/Modification" form (Form <br />C) shall be submitted by the owner or the owners'agent [pursuant to (CCR) 2635(d)) <br />before this upgrade will be considered complete by EHD. <br />10. Submit to EHD an updated Monitoring Plan and Tank Information form (Form B) within <br />30 days of completion of work. <br />11. Permit expires 180 days from date of approval.
