An approved <br /> copy will be returned for on site records. <br /> 8. A"UST Certification of Installation/Modification"form shall be submitted by the owner or <br /> the owner's agent (CCR Section 2635(d)) before this upgrade is considered complete by <br /> EHD. <br /> 9. Conduct secondary containment test for all newly installed UST components 6 months <br /> after installation. <br /> 10. All components must be certified by an independent testing organization and labeled as <br /> required by the organization (UL, ULC, ANSI, API, ASTM, etc.). See California Code of <br /> Regulations (CCR) Title 23, Chapter 16, Table B for a complete list. <br /> 11. Submit to EHD a copy of the test report within 30 days of the tests, and a copy of each <br /> manifest signed by the receiving permitted facility of the hazardous waste (i.e. rinsate) <br /> generated. <br /> 12. Permit expires 180 days from the date of approval.
