DECONTAMINATION <br />A decontamination station and appropriate decontamination procedures shall be <br />established by the SHO prior to the beginning of operation. The decontamination <br />station shall be situated up -wind and up -gradient form the exclusion zone, be clearly <br />marked, and provide controlled access point for movement between the exclusion <br />zone and support zone. Decontamination requirements shall be reviewed with field <br />personnel during daily Tailgate SAft Meetings. The following decontamination <br />procedures shall be implemented: <br />LEVEL D <br />*Move to designated Decontamination area. <br />*Clean work boots of any accumulation of soil or mud. <br />*Remove leather gloves. <br />*Wash hands and face. <br />11 1 <br />The SHO shall conduct a Tailgate Safety Meeting with all field personal including sub- <br />contractor personnel, at the beginning of every shift. The tailgate Safety Meeting form <br />shall be updated whenever daily site conditions, hazards or scope of work changes. <br />This form shall be maintained on-site during the project. <br />F— INSPECTIONS <br />The SHO shall perform a daily inspection of the work site. The daily safety inspection <br />shall de documented using the Safety Inspection form. The SHO shall ensure that all <br />immediate hazards are corrected before work proceeds and that all other hazards and <br />potential safety situations are corrected in a timely manner in relation to this project. <br />�M" a 1►►/ U <br />In the event of an injury, illness or near miss, the incident shall be immediatly reported <br />to the Field Supervisor of SHO. If required emergency medical care or first aid be <br />rendered. The Field Supervisor shall then initiate a Supervisors Report of Injury, and all <br />other documents required for injury/illness reporting and worker's compensation claims. <br />As soon as possible after the occurrence of an occupational accident the SHO shall <br />initiate an Accident Investigation. The investigation shall be documented on a Accident <br />Investigation form. <br />Page 8 <br />E <br />
