Once is has been determined that the emergency no longer exists, the emergency <br />coordinator must provide for the treating, storage or the disposing of all recovered <br />waste, contaminated soil or debris or surface water form the release or emergency, <br />and ensure that all emergency equipment is cleaned and ready for its intended use, <br />before operations at the facility or plant are resumed. Within 15 days of the <br />emergency, the facility or the owner, or operator of a treatment, storage or disposal <br />facility shall submit a written report to the Regional Administrator in the EPA region <br />where the emergency took place... and provide the following information. <br />*The name, address and telephone number of the generator, owner or operator. <br />*The name, address and telephone number for the facility, plant or location, if <br />different than that of the generator, owner or operator. <br />*The date, time and type of incident. <br />*The name and quantity of the materials involved. <br />*The extent of any injuries or fatalities, if any. <br />* An assessment of the actual potential hazards to human health or the environment, <br />if applicable. <br />*And the estimated quantity and disposition of recovered materials that resulted form <br />the incident. <br />Page 14 <br />11 <br />
