See `Sampling Protocol For Routine Piping Removals' and `Verification Analysis' on <br /> page 6 and 7 of the permit application for required analytical tests and number of <br /> samples to be taken and witnessed by EHD. Please be aware UST piping shall not be <br /> removed until EHD is present and sampling locations have been identified. <br /> 8. In addition, the following information must be submitted within 30 days after the UST <br /> removal date: <br /> a. Only analytical results that come directly from a third party California certified <br /> laboratory with affixed chain of custody will be accepted by this EHD. <br /> b. UST Disposition Tracking Record Sheet and/or Tank Hazardous Waste Manifest. <br /> c. If applicable, Hazardous Waste Manifest for rinsate, residual fuel, or waste oil <br /> receipt. <br /> d. If applicable, submit the backfill excavation certificate as required by EHD <br /> 9. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all of the <br /> following: <br /> a. One of several permissible valid, current contractor's licenses issued by the <br /> Contractors State License Board (may be issued to the installation company), or a <br /> Tank Tester's license issued to the individual by the State Water Board, and <br /> b. A valid, current California UST Service Technician certificate issued by the <br /> International Code Council (ICC), and <br /> c. A valid, current certificate of training from the manufacturer(s) of the component(s) <br /> being tested, repaired, or serviced. <br /> 10. Qualified installers must possess or work under the direct and personal supervision of an <br /> individual physically present at the work site who possesses all of the following: <br /> a. A valid, current contractor's license issued by the Contractors State License Board, <br /> and <br /> b. A valid, current certificate of training from the manufacturer(s) of the component(s) <br /> being installed, and <br /> c. A valid, current UST Installation/Retrofitting certificate issued by the International <br /> Code Council (ICC). <br /> 11. Permit expires 180 days from the date of approval.
