No testing of the annual Monitoring System Certification was performed. <br />Facility has no UST operator since December 2010 and has been monitored daily since December 22, <br />2010 until June 30, 2012 by Savemart Management. Temporary closure procedures were not <br />performed. SSR Group Inc lease agreement starts July 1, 2012. <br />104. Financial responsibility documents have not been submitted to the EHD. No financial responsibility <br />document received at this site for 2010, 2011 and 2012 year. Last financial responsibility document <br />received was from August 2009 from the previous operator who was evicted from the facility in <br />December 2010. Current financial responsibility documents are required to be submitted annually. <br />Complete and submit a copy of the financial responsibility. <br />105. UST facility (Form A) and tank forms (Form B) on file with the EHD are not current. Any change of <br />information on these forms must be submitted to the EHD within 30 days of the changes. Per Savemart <br />Management Company, SSR Group Inc will begin lease take over for this facility in July 2012. <br />Immediately complete and submit a copy of the facility and tank forms to the EHD. Complete a separate <br />tank form for each tank and submit copy of Form A & B. <br />105. UST facility has not submitted a BOE Maintenance Fee Account Numbers. Contact California <br />State Board of Equalization to apply for all necessary tax ID numbers necessary to operate business. <br />Immediately complete and submit a copy of the facility form to the EHD. <br />106. The monitoring and response plans on site are not current and not approved by the EHD. The <br />monitoring and response plans must be current and approved by the EHD. Complete and submit a copy <br />of the monitoring and response plans for approval by the EHD. An approved copy will be returned for <br />your records. <br />108. Secondary containment testing was last performed on July 29, 2008 and was due by July 2011. <br />Testing has not been completed and is 12 months past due. Secondary containment testing is required <br />once every 36 months.
