Annual monitoring system certification, leak detector testing, and spill container testing <br />were last performed on August 31, 2009 and was due by August 2010. Testing has not been completed <br />and is almost two years past due. Test results from August 31, 2009 was never submitted to EHD. <br />These tests are required once every 12 months. Immediately schedule these tests and provide 48 hours <br />notification to the EHD. Please be aware that a follow up inspection will be charged at our current hourly <br />rate. <br />119 & 317. A permit was not obtained from EHD to temporary close. Savemart Management had an <br />agreement with EHD to monitor the site on a daily basis since the eviction of a former operator in <br />December 2010. UST were not in use since approximately July 2010 by SJVAPCD due to failure of <br />upgrading EVR system. Savemart has handled over the property to SSR Group Inc beginning of July <br />2012. It is unclear if the new operator intends to return the UST back to use or when it will be returned <br />back to use. No person shall abandon an UST system without complying with the requirements of HSC <br />25298. Immediately obtain a permit from the EHD to temporarily close this UST system. <br />116. The UST system at this facility has been identified by the State Water Resources Control Board to <br />be located within 1000 feet of a public drinking water well and must test the UST system once using an <br />enhanced leak detection test. This testing has not been completed. Immediately schedule this test and <br />provide 48 hours notification to the EHD. Provide copies of the test results within 60 days after test has <br />been performed. <br />204 & 205. Designated operator has not been identified for this facility and monthly visual inspections <br />has not been performed. Last designated operator report for this facility was on August 31, 2009. There <br />has not been any monthly visual inspection reports since then nor is there a current designated operator <br />for this facility. The designated operator duties includes documenting all the alarms from the attached <br />alarm history on each of the designated operator monthly inspection report and check that they were <br />responded to appropriately. Designated operator must document action taken in response to any <br />alarms. They must visually inspect the UST system for leaks, water, or debris during the monthly <br />inspection including spill containers and UDC sumps, and any tank top sump that had an alarm for which <br />there is no record of a service visit. Ensure that designated operators performing monthly inspections at <br />this facility are including all of the required information on the reports. Monthly visual inspection reports <br />must be on site for a minimum of 12 months. Submit to EHD an Owner Statement identifying who is the <br />current designated operator.
