This information was required to be submitted by August 29, 2015. This <br />information was received at the Environmental Health Department (EHD) on October 27, 2015, approximately two <br />months late. An operator that receives an inspection report shall have 30 days to submit a written response that <br />includes a statement documenting corrective actions taken or proposing corrective actions which will be taken. <br />Ensure that a written response documenting corrective actions taken or proposed is submitted within 30 days of <br />receiving an inspection report. <br />This is a repeat violation, Class 11. <br />201 <br />CCR 2712(b) Failed to maintain alarm logs and/or records of follow up actions. <br />Maintenance and monitoring records for the last three years were not found on site. Maintenance records from 2013 <br />were not available for review during the inspection. Daily monitoring records from 2013 through July 2016 were not <br />available for review during the inspection. These records shall be maintained on site for at least three years. <br />Monitoring records include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and <br />maintenance records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the <br />field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) <br />results of inventory readings and reconciliations. Immediately locate and maintain all maintenance and monitoring <br />records for the last three years on site and submit copies to the Environmental Health Department (EHD). <br />This is a repeat violation, Class II. <br />203 <br />CCR 2712(b) Failed to maintain records of monitoring, repair, lining, and upgrades on site. <br />The monitoring system certification report for testing completed on July 30, 2015, was not on site during the <br />inspection. Records of repairs, linings, and upgrades for the UST system were not found on site for the year 2013. <br />These records shall be kept on site for the life of the tank. Immediately locate and maintain these records on site and <br />submit copies to the Environmental Health Department (EHD). <br />This is a repeat violation, Class II. <br />SEP 01 2016 <br />Page 4 of 6 <br />
