Instead the International Code Council operator's <br />certification expiration date should be December 4, 2016. The October 2015 designated operator monthly report was <br />not available for review during the inspection. The August 3, 2015 designated operator monthly inspection report lists <br />Curtis Carpenter as the designated operator with a certified operator expiration date of March 3, 2015, indicating that <br />the operator's certification had expired five months prior to the designated operator's monthly inspection in August. <br />During the monthly inspection, the designated operator shall check that all required information is current and correct. <br />Ensure that designated operators performing monthly inspections at this facility are including all of the required <br />information on the reports. <br />This is a repeat violation, Class II. <br />210 <br />CCR 2715(e) Designated operator monthly inspection reports not maintained on site for at least 12 months. <br />The October 2015 designated operator monthly inspection reports was not found on site. Designated operator <br />monthly inspection reports for the previous twelve months shall be retained on site. Locate and ensure that copies of <br />the previous twelve months of designated operator monthly inspection reports are maintained on site. Submit copies <br />to the Environmental Health Department (EHD). <br />This is a minor violation. <br />315 <br />HSC 25291(e) Water in secondary containment not removed, analyzed, and properly disposed of (pre -Jul 2003). <br />Liquid was observed in the 87 -product and 91 -product submersible turbine pump (STP) sumps and the 89 -product and <br />91 -product fill sumps. Approximately one cup of water was removed from the 87 -product STP sump; approximately <br />one quart of water was removed from the 91 -product STP sump; approximately one cup of water was removed from the <br />91 -product fill sump, and approximately two cups of water were removed from the 89 -product fill sump. if water could <br />enter into the secondary containment by precipitation, or infiltration, it must be removed and disposed of properly. The <br />service technicians removed the liquid from the sumps. <br />This was corrected on site. <br />This is a repeat violation, Class II. <br />319 <br />CCR 2635(b) No spill bucket, failed to contain five gallons, or failed to provide a means to keep it empty. <br />The 91 -product spill container failed when tested. All spill containers shall have a minimum capacity of five gallons <br />and be capable of containing a spill or overfill until it is detected or clean up. The spill container was replaced with an <br />OPW spill container P761C-EVRDV and an Underground Storage Tank Retrofit Verification with Inspector Already <br />Onsite form was completed by the inspector.
