Financial responsibility documents have not been submitted to the Environmental Health <br />Department (EHD). Current financial responsibility documents are required to be submitted annually. <br />Complete and submit a copy of the financial responsibility by June 18, 2012. <br />112. The diesel piping sump liquid sensor did not turn off the turbine when tested and therefore does not <br />meet the requirements for the annual line tightness testing exemption. The diesel turbine relay was not <br />working correctly. <br />Annual line tightness testing is required unless the UST system is equipped with the following.. <br />1. A continuous monitoring system that either activates an audible and visual alarm or stops the flow <br />of product at the dispenser when it detects a leak <br />2. A monitoring system that shuts down the pump or stops the flow of product at the dispenser when <br />it detects a leak <br />3. A monitoring system for piping other than that in UDC sumps (tank top sumps, transition vents, <br />etc.) is fail safe and shuts down pump when a leak is detected <br />Immediately correct by installing a new diesel turbine relay OR schedule a line test and provide 48 hours <br />notification to the EHD. Apply for a repair permit with the EHD if the relay is replaced. <br />113. A new designated operator, Atila Gil Fernandes, was hired in 2011 and notification was not <br />provided to the EHD within 30 days of the change. Any changes shall be reported to the EHD with 30 <br />days. Submit notification to the EHD identifying all the designated operators for this facility by June 18, <br />2012. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT END'S CURRENT HOURLY RATE. <br />EFI�2'-Rev o9/oV11 Page 3 UST INSPECTION REPORT <br />
