The designated operator failed to check and note the correct test date for the triennial secondary <br />containment test on the April 30, 2012 designated operator monthly inspection report. The correct <br />secondary containment test date is October 14, 2010. During the monthly inspection, the designated <br />operator shall check that all required testing and maintenance for the UST system have been completed <br />and document the dates they were done. Ensure that designated operators performing monthly <br />inspections at this facility are including all of the required information on the reports. <br />204. The October 2011 monthly designated operator inspection was done on November 2, 2011, two <br />days late. Ensure that the designated operator inspection is done once every calendar month. <br />206. The designated operator employee training for Bryant was not performed within the past 12 <br />months. The designated operator shall train facility employees for which he or she is responsible in the <br />proper operation and maintenance of the UST system once every 12 months. The training shall include, <br />but is not limited to: <br />1. Operation of the UST system in a manner consistent with the facility's best management practices <br />2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br />3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br />4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br />Ensure that employees have been trained by the designated operator, maintain the list on site, and <br />submit a copy of the training records to the EHD by June 18, 2012. <br />305. The waste oil tank sump liquid sensor failed to activate an audible and visual alarm when tested to <br />ensure continuous monitoring of the tank system. All monitoring equipment shall be maintained to <br />activate an audible and visual alarm when it detects a leak. Correct immediately by having a properly <br />licensed, trained, and certified contractor replace the failed component with a functional component (LG <br />113-x listed, if applicable) and obtain a permit within 1 business day from the EHD. If the failed <br />component can't be replaced immediately, there is a possibility that the waste oil UST system may be <br />red tagged to prevent waste oil inputs. <br />Notes <br />1. Complete the following by June 18, 2012 and send to the San Joaquin County Environmental Health <br />Department: <br />a. The Return to Compliance Certification form. <br />b. A statement explaining how each violation was corrected. <br />c.
