The 2014 Monitoring System <br />Certification test results were not found onsite. These records shall be maintained on site for at least three years. <br />Monitoring records include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and <br />maintenance records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the <br />field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) <br />results of inventory readings and reconciliations. Immediately locate and maintain all maintenance and monitoring <br />records for the last three years on site and submit copies to the EHD. A copy of the 2014 Monitoring System <br />Certification test results were emailed during the inspection. This was corrected on site. <br />This is a repeat violation, Class II. <br />209 <br />CCR 2715(f) Designated operator employee training not performed or log not kept. <br />The designated operator employee training for Bob's Lube & Oil facility was not performed. The designated operator <br />shall train facility employees for which he or she is responsible in the proper operation and maintenance of the UST <br />system once every 12 months. The training shall include, but is not limited to: <br />1. Operation of the UST system in a manner consistent with the facility's best management practices <br />2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br />3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br />4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br />Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of <br />the training records to the EHD. <br />This is a repeat violation, Class II. <br />309 <br />CCR 2636(f)(1) Monitoring in the UDCs failed to activate an audible and visual alarm or stop the flow at the UDC. <br />The Bravo box float and chain in the on the 91 in the 3-4 UDC sump failed to stop the flow of product at the dispenser <br />when tested. All monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow of <br />product at the dispenser when it detects a leak. The service technician adjusted the chain(s) and verified functionality. <br />This was corrected on site. <br />This is a repeat violation, Class II. <br />319 <br />HSC 25291(e) Water in secondary containment not removed, analyzed, and properly disposed of (pre -Jul 2003). <br />Liquid was observed in the diesel STP sump .If water could enter into the secondary containment by precipitation or <br />infiltration, it must be removed and disposed of properly. The service technician removed three cups of liquid from the <br />diesel STP. sump. Ensure that all sumps and annular spaces are maintained free of liquid.
