Submit 3 complete sets of drawings illustrating the locations of all existing anti proposed <br />structures, including existing TUST(s), in non -erasable pant to pHS-EED. Upon approval, <br />one set will be retained by PHS-EHD and the other two will be Leturaed. <br />b. Include equipment and material specificaWn sheets and mazsuiactLxrex's brochtirtes describing <br />field testins procedures, operating and monitoring systems and listing numbers or other <br />evidence of a t~tationally recognized testing orgardzation approval. All proposed leak <br />detection moEhods and equipment roust meet the requirements specified in 40 CFR 280.44 <br />and have been reviewed by the State Water Resources Control Board Staff. <br />All interstitial monitors roust perform according to manufacturer's =ndards, and must be <br />tested annually in accordance with section 26300 (d), California Code of Regulations (CCR), <br />Title 23 Waters, Division 3, Chapter 16. <br />d, Include a detailed monitoring and response plan in accordance with section 2632 (d) (1) 8c <br />{2}, CCR, Title 23 Waters, Division 3, Chapter 16. <br />e, The plates sbould include a cross sectional diagram of the tank and.piping system inciudinm <br />secondary conminmeac, overflow protection equipment, and extension or an pipes as &or <br />ports from below grade to finish grade. <br />f, Submit a completed "FHS -E HD Service Request" form and. the State Water RescLrces <br />Control Hoard "Facility Permit to}" and "Tank Permit (B)" application forms_ <br />SITE miTICATION: <br />Ln the event contamination is observed., coeirmed or suspected as a result of a leaking UST <br />system it is the responsibility of the owner or operator to submit a workplan to PHS -EM <br />Sire Mitigation emit prior to iaitiadlIg any assessment or remediation activid-es.
