11-30-1999 2: 55PM FROM <br />New facility plan review fee of 5624.00 (8 hour rninimurn per facility), $170.00 fee per <br />tank, plus $56.00 state surcharge fee per tank is due at time of pian submittal. <br />b. New installation of an additional tank at an existing facility is $624.00 pian review fee (8 <br />hour minimum. per facility) Plus $170.00 fee per new tante, and $56.00 per tank state <br />surcharge fee. <br />P. 2 <br />It should be noted that a $524.00 plan review check fee required for each submittal allows <br />for up to eight (8) hours of staff review and/or inspection time. Additional time spent on <br />plan review. meetings, i-:sp=lions, phone or office consultations will be billed to the <br />applicant at a rare of S78.00 per hour. <br />d. 'llhe maximum review time for UST installation plans is 40 worliag days_ A preliminary <br />review will be conducted by the PHS-EHD within 20 working days of submittal date to <br />identify any gross plan deficiencies. <br />e. If gross deficiencies are identified, the plans will be returned with a checklist identifying plan <br />deficiencies. <br />To expedite the review of installation plans, time and a half per hour overtime fee may be <br />submitted to have review completed within 10 working days of submittal date if staff is <br />available to perform the review. <br />tr tsl� <br />The enclosed pian, review sheet is to be complered and sulm itred with the three sets of <br />drawings to PHS-ERD. <br />b. A copy of the California UST Regulations may be obtained from he California State Water <br />Resources Control Board at (316) 227-4303. <br />C. Pamirs andlor plan check approval may also be required by the San Joaquin Valley Unified <br />.Air Pollution Control District, the Corny/City Building Department and the Fire District ia <br />your respective area. <br />d. A minimum of 3 inspections will be conducted on the UST system installation_ A $78 per <br />hour inspection fee shall be charged for additional inspections or reinspection as recurred. <br />(See required inspections below) <br />Applicant/contractor to adhere to the approval plant and any charge or deviation shall be <br />reported to and approved by PHS-EHD prior to installation. <br />f, The applicatzou for installation of underground tants is only valid for the calendar year in <br />=which it has been isTued. A permit may be extended into the next calendar year if a letter is <br />sent to PISS-EHD requesting this extension thirty days prior to the end of the calen&r year. <br />A. one tame, one year extension my be granted by PHS-EHD upon receipt of this letter. <br />2 <br />
