The guidelines specify the consu%=twn and moairrrLm criteria <br />necessary to complete our review of your proposed underround storage tank (UST) systeut. <br />1, SU,BMTA,.L OF PIANS <br />INSTALLATION. <br />Submit 3 complete sets of drawings illustrating the locations of all existitg and proposed <br />structures, including- existing UST(s), in non -erasable print to PHS-EEID. T�pon approval, <br />one set will be retained by PHS-EHD and the other two will be returned. <br />o. include equipment and material specification sheets and manufacturer's brochures describing <br />field testing procedures, operating and monitoring systems and listing aumbers or other <br />evidence of a nationally recognized testing organization approval. All propos„-d leak <br />detection methods and equipment must meet the requirements specified. in 40 CFR 284.44 <br />and have been reviewed by the State Water Resources Conuoi Board Staff. <br />All interstitial 1-norimrs must perform according to manufacturer's standards, and must be <br />tested annually in accordance with section 2,630 (d), California Code of Regulations (CCR), <br />Title 23 Walrus, Division 3, Chapter 16. <br />d, Include a detailed monitoring and response plan in accordance with section 4632 (d) (1) & <br />(2), CCR, Tide 23 Waters, Division 3, Chapter 16. <br />e. The plans should include a cross sectional diagram of the taint and piping system including <br />secondary containment, overflow protection equipment, and extension of all pipes and/or <br />ports from below grade to finish grade. <br />Submit a Completed PHS-EHD Service Request form and the State water Resour=s <br />Control Board "Facility Permit (A)" and "Tank Permit (B)" application forms_ <br />SITE MITIGATION- <br />a. in the event contamination is observed, confirmed or suspected as a result of a leaking UST <br />system it is tete responsibility of the owner or operator to submit a workplan to PHS -EM <br />Sim Mitigation Vuit prior to initiating any assessment or remediation activities. To minimize <br />deiays, a Corrective Action Plan may be submitted concurrent with tank closure oz <br />installation plans. Address one copy of the Corrective Action Plan to the Site Mitigation <br />Unit for review.
