Permits andlor plan check approval may also be required by the San Jbaquin Valley UWfied <br />Air Pollution Control District, the Counry�Ciry Building Department and the Fire District in <br />your respective area. <br />d. A minimum of 3 inspectons will be conducted on the UST system installation- A $78 per <br />hour inspection fee shall be charged for additional inspections or reinspection as required. <br />(See required inspections below) <br />e. Applicant/contractor to adhere to the approval plan and any change or deMation shall be <br />reported to 1W approved by PHS-EHD rmior to installatiori, <br />f. The application for installation of undergrotizd tanks is only valid for the calendar year in <br />which it has been issued. A permit may be extended into the next calendar year if a letter is <br />sent to PHS-EHD requesting this cxterision thirty days prior to the end of the calendar year. <br />A one time. one year extension may be granted by PHS-EHD upon receipt of this letter. <br />
