Only analytical results that cWcusitodire y from a third party California certified <br /> laboratory with affixed chaindy ill be accepted by this EHD. <br /> b. Hazardous Waste Tank Clo ure Certificat n Form (DISC Form 1249) <br /> c. Hazardous Waste Manife for rinsate and r sidual fuel and ethylene glycol waste/ <br /> waste from interstitial sp ce. <br /> 12. Permit expires 180 days f m the date of approval.
