Our Return to Compliance Certificate is <br />enclosed. Kaiser has addressed the issues with the Underground Storage Tank System at the Manteca Medical Center as follows: <br />Item 2: Secondary Containment testing <br />On October 21, 2009, Kaiser's subcontractor R&D Compliance conducted testing of its secondary containment system <br />immediately following Kaiser's replacement of the two un -permitted pipes, and its repair and replacement of the secondary <br />containment return line and piping sump. Kaiser takes full responsibility for its inadvertent failure to re -test the system within six <br />months following this installation. The secondary containment system passed testing performed in late 2010. <br />To ensure that future testing is completed and submitted to SJCEHS in a timely manner, Kaiser has created a work order <br />in our Electronic Preventive Maintenance System that requires an annual review of the UST system and its associated equipment. <br />This work order includes a review of the previous inspection and the associated record documentation, as well as a check of the last <br />date of tri -annual inspection. Any deficiencies resulting from our reviews will be documented and tracked by the same work order <br />system. <br />As you are perhaps aware, Kaiser Management has recently determined that the Manteca Medical Center will transition <br />fuel storage to an aboveground storage system. Decommissioning of the UST system will be the subject of separate <br />communications with SJECEHS, and will be performed in accordance with all legal requirements. <br />Item 3: UST Tank Forms on File <br />Manteca Medical Center has updated the UST Operating Permit Application after your review on August 3, the date of <br />inspection. The Manteca Medical Center will notify the SJCEHS department if there are any changes in information or work that <br />need to be addressed to the Underground Storage System <br />Item 40: Daily Checks on Leak Monitoring System <br />At the time of the SJCEHS inspection, the Manteca Medical Center did not have an engineering position on call to address <br />the building and equipment rounds for holiday and sick coverage. Immediately following the inspection, Manteca Medical Center <br />administration has authorized the Engineering Department to backfill the vacancy on holidays or sick days with an employee to <br />perform equipment rounds, including monitoring operation and inspection of the underground fuel storage system. There have <br />been no unfilled vacancies since the SJCEHS inspection. <br />Kaiser's Return to Compliance Certificate includes our certification and relevant documentations.
