CASE SUMMARY <br />DIRECTOR <br />Donna Heran, REHS <br />PROGRAM COORDINATORS <br />Margaret Lagorio, REHS <br />Robert McClellon, REHS <br />Jeff Carruesco, REHS, RDI <br />Kasey Foley, REHS <br />This is an underground storage tank (UST) facility consisting of one 8,000 -gallon tank <br />containing diesel for an emergency generator at Kaiser Permanente Hospital, 1777 W. <br />Yosemite Avenue, Manteca, CA. <br />On August 4, 2005, a permit application, SR0043375, was submitted to replace the <br />diesel spill container and mechanical overfill prevention device. Ms. Michelle Le, Senior <br />Registered Environmental Health Specialist (REHS), San Joaquin County Environmental <br />Health Department (EHD), approved the permit the same day (Attachment 1). Ms. Le <br />witnessed the testing of the new spill container on September 6, 2005. <br />On October 10, 2005, Mr. Dennis Catanyag, Senior REHS, EHD, sent a letter to the <br />facility stating that the annual monitoring system certification, spill container testing, and <br />line tightness testing was last performed on August 20, 2004, and were past due <br />(Attachment 2). <br />On November 18, 2005, Mr. Catanyag performed a routine UST inspection (Attachment <br />3) and witnessed the annual monitoring system certification, spill container testing, and <br />line tightness testing. Testing was performed three months past due, and the tank <br />tightness testing that was required as a condition of permit SR0043375 was not <br />performed. A report was submitted, though it incorrectly listed the tank size as 10,000 - <br />gallons (Attachment 4). <br />On February 13, 2006, Mr. James Brakkee, Chief Engineer, Kaiser Permanente <br />(Kaiser), submitted a return to compliance certification that stated line tightness testing <br />was performed after the spill container was replaced (Attachment 5).
