Mr. <br />Brakkee stated he would schedule the tests. <br />On April 7, 2006, secondary containment testing was performed and a test report was <br />zubmitted to the EHD (Attachment 6). All components passed. <br />On August 30, 2006, Mr. Muniappa Naidu, Senior REHS, EHD, performed a routine UST <br />inspection (Attachment 7) and witnessed the annual monitoring system certification and <br />spill container testing. Annual line tightness testing was not performed this day, but Mr. <br />Naidu indicated in the report that it would be due by November 2006. No violations were <br />noted and all components tested passed. A test report, due thirty days after testing, was <br />never submitted. <br />On August 30, 2007, Mr. Naidu performed a routine UST inspection (Attachment 8) and <br />witnessed the annual monitoring system certification and spill container testing <br />(Attachment 9). Annual line testing due by November 2006, was not done, though Mr. <br />Naidu did not indicate that on the inspection report. During a review of the facility's files, <br />Mr. Naidu found that a new designated operator had been hired but notification was not <br />provided to the EHD. <br />On August 28, 2008, Mr. Naidu performed a routine UST inspection and witnessed the <br />annual monitoring system certification and spill container testing. Mr. Naidu noticed tha) <br />there appeared to be newly poured concrete around the tank. Upon closer inspection, <br />two new pipes were found installed through the piping sump without any penetration <br />fittings. Mr. Brakkee stated he did not know about the work done on the LIST system. <br />Mr. Naidu included a Notice to Abate Immediately on the inspection report to stop work <br />on the UST system and to submit a permit application for approval (Attachment 11). A <br />test report, due thirty days after testing, was never submitted. <br />P11 <br />
