Failing to maintain a copy of the monthly inspection record and all attachments for <br />the previous twelve months, in violation of California Code of Regulations, title 23 section 2715(e). <br />4. Defendant, in the course of Defendant's HOSPITAL activities is, pursuant to Business and <br />Professions Code § 17204, hereby permanently enjoined from: <br />a. Violation of Health and Safety Code section 25280 et seq. described in paragraph <br />3 - 3k above; <br />5. Defendant shall pay to Plaintiff the sum of TWENTY-TWO THOUSAND FIVE <br />HUNDRED DOLLARS ($22,500.00), as and for civil penalties, pursuant to Health and Safety Code <br />§25299. Said payment shall be made Rlyable to the SAN JOAQUIN COUNTY <br />ENVIRONMENTAL HEALTH DEPARTMENT - ENVIRONMENTAL HEALTH DIVISION <br />TRUST. Said payments shall be delivered to the San Joaquin County Office of the District Attorney, <br />Environmental Prosecutions Unit, at time of signing. <br />6. Defendant shall pay to Plaintiff the sum of TWENTY-TWO THOUSAND FIVE <br />HUNDRED DOLLARS ($22,500.00), as and for civil penalties, pursuant to Business and <br />Professions Code section 17206. Said payments shall be made payable to the TREASURER OF <br />SAN JOAQUIN COUNTY and delivered to the San Joaquin County Office of the District Attorney, <br />Environmental Prosecutions Unit, at time of signing. <br />7. Defendant shall fund Environmental Training Programs in the amount of FORTY-FIVE <br />THOUSAND DOLLARS ($45,000.00), as special projects, commonly known as Supplemental <br />Environmental Projects, as described below. The payment shall be due and payable to the below - <br />mentioned agencies, and shall be delivered to the San Joaquin County Office of the District <br />Attorney, Environmental Prosecutions Unit, at time of signing. The FORTY-FIVE THOUSAND <br />DOLLARS ($45,000.00) will be allocated as follows: <br />a. TWENTY THOUSAND DOLLARS ($20,000.00) payable to the CUPA Forum <br />Environmental Protection Trust Fund to be used by the San Joaquin County Environmental Health <br />Department and the San Joaquin County Office of the District Attorney for the purpose of <br />participating in training courses which are designed to enhance the investigation, inspection and <br />enforcement of Unified Programs throughout the State of California; <br />3 <br />PERMANENT INJUNCTION AND FINAL JUDGMENT PURSUANT TO STIPULATION <br />
