AmokAE�M AW-1- <br />Frain �111�'"Inow a %W t M1111111W <br />To: Mr. Ray von Flue, E.H.S. <br />From: John Lynch, Wright Environmenal Services, Inc. <br />RE: 7373 West Lane, Stockton CA <br />The memo dated December 9, 2002 from PHS-EHD noted certain deficiencies in Service Request No. <br />0032058 dated December 3, 2002 for repairs at Kaiser Permanente in Stockton (Operator) at the <br />referenced address. The Service Request submitted was for the testing and repairs noted in that <br />request During the testing covered by the original Service Request, additional repairs and <br />improvements to the Backup Fuel Supply System were identified. Discussions with PHS-EHD resulted <br />in Wright and PHS-EHD acknowledging that an amendment to the original Service Request was <br />required to describe and incorporate the additional work into the Service Request previously submitted. <br />Prior to the Service Request of 12-3-02, Wright had coordinated with Mr. Wilson at PHS-EHD to provide <br />notice of SB -989 testing for the back up fuel system. <br />Testing of the of the four existing fiberglass secondary containment piping runs resulted in three failing, <br />and, one passing a 5 -psi test. Two existing black iron supply (safe suction) and two return lines <br />between the tank piping sump and the transition fittings where the supply and return lines transition to <br />above grade piping in the boiler and generators rooms were tested. Additional tests were initiated to <br />attempt to isolate the leaks in containment piping. These tests did not isolate the leaking secondary <br />containment piping. No leaks were observed in the primary supply and return lines and no diesel fuel <br />was observed within the containment piping or piping sump. A copy of the secondary containment <br />piping test and protocol is attached (Attachment 1). <br />Tank Testing (Annular & Primary) <br />Operator requested Wright to repair annular riser that had an improper coupling (rubber coupling) seven <br />inches below test fitting within grade ring. This rubber coupling connected the steel riser from the tanks <br />annular space to a PVC nipple that extended the annular space riser to within 4-inchs of grade for <br />testing. Apparently when the operators testing firm tested the primary tank and the annular space the <br />annular space vacuum test failed. The failure was attributed to the rubber coupling and Wright was <br />then hired to make the repair under the original Service request submitted. After that repair to the <br />annular space riser, Wright contracted with Afford -a -Test and the tanks primary and annular space <br />passed testing. Copies of the Certification from Afford -a Test are attached for your records (Attachment <br />2). If an additional test is required, please advise so that this can be rescheduled with the certified <br />tester. <br />
