Manthey Dr <br />Stockton CA 95206 <br />The San Joaquin County Environmental Health Department (EHD) has completed the <br />review of the application for permit (SR0052704) to conduct the following work: <br />• Replace leak detector (87) <br />The permit has been approved with the following conditions: <br />1. Contact the Fire Department and San Joaquin Valley Air Pollution Control <br />District for applicable permits. <br />2. Contact Michelle Henry (209) 953-7699 to arrange the following inspection <br />appointments (a 48-hour notice is required for inspection appointments): <br />a. Verify functionality of 87 leak detector, i.e. will detect leak of 3 <br />gallons per hour at 10 PSI. <br />3. Qualified service technicians must possess or work under the direct and <br />personal supervision of an individual physically present at the work site who <br />possesses all of the following: <br />a. One of several permissible valid, current contractor's licenses issued <br />by the Contractors State License Board (may be issued by the <br />installation company), and <br />b. All valid, current California UST certificates issued by the <br />International Code Council (ICC), that are applicable to the work <br />being performed, and <br />c. A valid, current certificate of training from the manufacturer(s) of the <br />component(s) being tested, repaired, or serviced, and <br />d. Each person installing or testing any component must hold a certificate <br />of training from the manufacturer of that component. <br />4. All components installed must be certified by an independent testing <br />organization and bear the appropriate mark as required by the <br />independent testing organization. <br />
