5. All monitoring equipment must be approved and listed in the State Water <br />Resources Control Board's Local Guidance Letter 113, "List of Leak <br />Detection Equipment and methods for Underground Storage Tanks." <br />6. Complete and submit a copy of a UST Installation Certificate of Compliance <br />(Former Form C) for the 87 and the diesel tank no later than 30 days after <br />work is completed. <br />7. This permit expires 90 days (January 5, 2008) from the approval date <br />8. Any changes in information and scope of work must be submitted to EHD for <br />approval prior to commencing work.
