Contact the fire department for any applicable permits. <br /> 2. Work performed regarding pipe removal/installation shall be minor, (i.e. less than <br /> 5 feet of piping involved)otherwise separate permits for removal and installation <br /> will be required. <br /> 3. Once the source of the secondary testing failure has been detected,contact PHS- <br /> EHD so that it can be determined whether soil sunpiing will be required. <br /> 4. Depending on the result of the soil samples, an addendum must be submitted <br /> regarding the proper handling of contaminated piping. <br /> 5. All pipes and pipe fittings shall bear the UL label. <br /> 6. All installation and testing shall be in compliance with manufacturer's instruction. <br /> 7. A hydrostatic test on primary.piping and a test compatible with product stored in <br /> primary piping for the secondary piping must be performed. <br /> 8. Contact Dennis Catanyag for inspection.
