Box 2292 Brea, CA 92821-2922 San Joaquin County Environmental Health Department 600 East Main Street Stockton, California 95202-3029 Inspector Naidu, This letter is in response to the inspection report issued on January 19, 2012 to Chevron Station#201761, located at 1103 S Main St. Manteca, CA 95337 Below is our response regarding the 4 items noted in the attached inspection report. ■ 105.The tank forms on file with the EHD are not current. Any change of information on these forms must be submitted to the EHD within 30 days of the changes. • Per the inspection report item#105 was address at the time of inspection. "A current copy of the tank forms for each tank was provided" ■ 202.Approved copies of the monitoring and response plans were not found on site. Copies of these plans shall be retained on site at all times, • Per the inspection report item#202 was address at the time of inspection. "A current copy was provided, approved and copy left on site" ■ 204. The designated operator failed to check and note the correct test dates for the monitoring certification and spill bucket test on the designated operator monthly inspection report from July, 2011 to January, 2012 is 1/31/11 and should be 1/21/11. During the monthly inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. Ensure designated operators performing monthly inspections at this facility are including all of the required information on the reports. • As stated on during our phone conversation, the designated operator has been coached and knows that the records must be reviewed monthly and the appropriate dates documented. 1IPage
