Michael Canlis Blvd., French Camp Program: UST <br /> SUMMARY OF VIOLATIONS <br /> CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> ROUTINE ANNUAL INSPECTION & MONITORING SYSTEM CERTIFICATION <br /> Violations: (6, 11, 13, 21) <br /> 6. A copy of the 2010 UST operating permit was not found on site. A copy of the UST operating permit <br /> and all attachments and conditions shall be retained at the facility. Immediately obtain a copy of the <br /> permit and retain it on site. <br /> 11. The 15,000 gallon (north) diesel annular sensor was found not located at the bottom of the tank to <br /> detect a leak at the earliest opportunity. The diameter was listed on paperwork on site as 119 inches. <br /> The riser was measured at 36 inches. The correct annular length was calculated to be 222.8 inches. <br /> The annular was found to be 190 inches. The 20,000 gallon (south) diesel annular sensor was found <br /> not located at the bottom of the tank to detect a leak at the earliest opportunity. The diameter was listed <br /> on paperwork on site as 119.5 inches. The riser was measured at 38 inches. The correct annular <br /> length was calculated to be 225.6 inches. The annular was found to be 218 inches. Once properly <br /> located, the 15,000 gallon (north) diesel annular sensor went into alarm due to liquid in the interstitial <br /> space. No actions were taken to address the alarm. Monitoring equipment shall be maintained to be <br /> able to detect a leak at the earliest possible opportunity. Immediately contact a properly licensed, <br /> trained, and certified contractor to address the alarm under permit and inspection of the EHD. Both <br /> annular sensors were relocated to the proper position by the technician at the time of inspection. <br /> 13. The 15,000 gallon (north) diesel sump sensor failed to activate an audible and visual alarm when <br /> tested to ensure continuous monitoring of the tank system. The sensor was found floating in water, but <br /> was not in alarm. No alarms were noted in the alarm log. All monitoring equipment shall be maintained <br /> to activate an audible and visual alarm or stop the flow of product at the dispenser when it detects a <br /> leak. Correct immediately by having a properly licensed, trained, and certified contractor replace the <br /> failed component with a functional component (LG 113-x listed, if applicable) and obtain a permit within <br /> 1 business day from the EHD. If the failed component can't be replaced immediately, there is a <br /> possibility that the 15,000 gallon (north) diesel UST system may be red tagged to prevent fuel inputs. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Ins ector: Received By: Title: <br /> i <br /> EHD 23 2-003 ev 08!10/10 CONTINUATION FORM <br />
