Contact the local fire department and San Joaquin Valley Air Pollution Control District for any <br />applicable permits. <br />2. Any changes shall be submitted to this office for approval prior to commencing work. All new <br />addenda supercede the previous ones. <br />3. At least one person who holds the International Code Council (ICC) UST Service Technician <br />certification and manufacturers' certificate of training for all components installed or tested must be <br />onsite when work is performed. <br />4. Each person installing or testing any component must hold a certificate of training from the <br />manufacturer of that component. <br />5. EHD must verify the overfill prevention system is in compliance with CCR 2635 (b)(2). Contact EHD <br />to schedule the following appointments: <br />a. Corrosion protection on the fill and annular riser prior to pouring concrete. <br />b. EHD must verify the overfill prevention system is in compliance with CCR 2635 (b)(2). <br />Note: Your company may choose to have EHD onsite during the overfill prevention <br />system installation so that portions of the system do not have to be <br />disassembled/reassembled. <br />c. The spill buckets must be tested to ensure they hold a minimum of 5 -gallons and pass a <br />one hour lake test or equivalent after pouring concrete. <br />6. Allow 48-hour notice for all appointments. <br />7. Update and submit a copy of the facility's Monitoring Plan to indicate the type of overfill prevention <br />present on each tank in Section IX. <br />8. A tank integrity test must be performed by a third party certified tank tester and results received by this <br />office within 7 days of completion of work. <br />9. Complete and submit a copy of a UST Installation Certificate of Compliance (Former Form C) for <br />each tank no later than 30 days after work is completed. <br />10.
