Harlan Rd, French Camp Program: UST <br /> SUMMARY OF VIOLATIONS <br /> (CLASS 1,CLASS 11,or MINOR-Notice to Comply) <br /> UST Inspection: Routine and Monitoring System Certification <br /> #23c. The designated operator failed to document the following alarms- October 1, 09 Mstrl-2 sIv 1, <br /> Master 3-4, Slave 2-3, Sens Loss of Signal, Power Up and Down,- October 13, 09 Power Up and Power <br /> Down; November 16, 09 Power Up and Down; February 7, 10 Power Up and Down; August 25, 10 <br /> System Fail Float Missing; September 23, 10 System Fail Float Missing. Failed to check that the alarms <br /> were responded to appropriately on the appropriate designated operator monthly inspection reports. <br /> During the monthly inspection, the designated operator shall review the alarm history for the previous <br /> month, check that each alarm was documented and responded to appropriately, and attach a copy of <br /> the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that <br /> designated operators performing monthly inspections at this facility are including all of the required <br /> information on the reports. <br /> 23c. The designated operator failed to check and note the complete test dates for the Monitoring System <br /> Certification, Spill Bucket testing and Secondary containment testing on the reports from July 2009 to <br /> present designated operator monthly inspection reports. Since July 2009, test dates are documented as <br /> Month and Year rather than Month, Day and Year. Documenting which day of the month will help the <br /> designated operator differentiate if an alarm had occur on a day a certification or a repair was done <br /> compared to a day no repairs or testing was done. During the monthly inspection, the designated <br /> operator shall check that all required testing and maintenance for the UST system have been completed <br /> and document the dates they were done. Ensure that designated operators performing monthly <br /> inspections at this facility are including all of the required information on the reports. <br /> 23c. The March 2010 designated operator monthly inspection report was not found on site. Designated <br /> operator monthly inspection reports for the previous twelve months shall be retained on site. Locate and <br /> ensure that copies of the previous twelve months of designated operator monthly inspection reports are <br /> maintained on site.
