The diesel spill container failed when tested during the one hour hydrostatic test. The fill cap was <br /> replace but the spill container continued to leak. An inflatable pressure ball was used and it was position <br /> below the drain valve but the spill container continued to leak. All spill containers shall have a minimum <br /> capacity of five gallons and be capable of containing a spill or overfill until it is detected or cleaned up. <br /> Immediately contact a properly licensed, trained, and certified contractor to repair or replace the spill <br /> container under permit and inspection of the EHD. Once the permit is approved, repairs to replace the <br /> spill bucket may begin. <br /> Submit to SJC EHD within 30 days (11/08/10): <br /> 1. one copy of the completed "Return to Compliance Certification" form <br /> 2. a written corrective action statement for the above violations <br /> 3. requested/supporting documents <br /> Notes: <br /> Secondary containment next tri-annual testing due by 12/2011. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHDIS CURRENT HOURLY RATE. <br /> EHD Inspe7r: Received By: Title: <br /> 4 <br /> - - 0�u_ <br /> EHD 2-003 R® 08/10/�10 CONTINUATION FORM <br />
