0 <br /> On December 3, 2008, a secondary containment test report for testing performed on November <br /> 20, 2008, was also submitted (Attachment 109). The report indicated failures at the annular <br /> space, piping sump, and secondary piping for the diesel product, and failures at the 87-octane <br /> secondary piping and the 91-octane piping sump. <br /> On December 10, 2008, return to compliance certification and supporting documentation was <br /> submitted in response to the November 6, 2008, routine UST inspection report (Attachment 110). <br /> According to the copy of the invoice submitted, approximately 37.5 gallons of water were pumped <br /> out of the diesel tank annular space. Financial responsibility documents submitted were <br /> incomplete and two alarms were not addressed. Mr. Backus sent a fax to Mr. Singh explaining <br /> the deficiencies. <br /> On December 11, 2008, a permit application, 5R0056071, was submitted by Elite IV to repair the <br /> secondary containment failures. The scope of work was inadequate and Mr. Backus sent a <br /> deficiency letter asking for a detailed scope of work(Attachment 111). <br /> On December 12, 2008, completed financial responsibility documents were submitted <br /> (Attachment 112). <br /> On December 18, 2008, an equipment list was submitted by Elite IV for permit application <br /> SR0056071. The list did not include all parts necessary to repair all of the failed components, <br /> since it could not be determined how repairs can be made until they are exposed. <br /> On December 31, 2008, a statement was submitted by Mr. Singh addressing the two alarms that <br /> were noted in the November 6, 2008, routine LIST inspection (Attachment 113). <br /> On January 23, 2009, Mr. Backus sent an approval letter to break concrete to expose the <br /> components that need repair with the condition that an update scope of work be submitted before <br /> actual repairs are made (Attachment 114). <br /> Attachment 115 includes parcel information for the Wilson Way, Charter Way, and West Lane <br /> facilities. <br /> Attachment 116 includes tank owner information (former Form A) for the Wilson Way, Charter <br /> Way, and West Lane facilities. <br /> Attachment 117 includes facility operator information for the Wilson Way, Charter Way, and West <br /> Lane facilities. <br /> Attachments 118, 119, and 120 include UST facility file narratives for the Wilson Way, Charter <br /> Way, and West Lane facilities, respectively. <br /> Attachments 121, 122, and 123 include hazardous waste facility file narratives for the Wilson <br /> Way, Charter Way, and West Lane facilities, respectively. <br /> 11.
