CLASS II, or MINOR -Notice to Comolvi <br />UST Inspection: Routine and Annual Monitoring System Certification <br />311. The 5 hspwr East Diesel UST VMI 99 LD -3000 mechanical line leak detector failed to detect a leak <br />when tested. East Diesel UST supplies fuel to the Truck Side. <br />-When testing the line leak detector, the operating pressure gauge read at the maximum pounds of 60 <br />but could be higher. Normal operating pressure is typically between 30 - 40 psi. <br />-The technician turned of the circuit breaker to the West Diesel turbine to ensure only one line leak <br />detector was operating. <br />-There is a switch in the breaker closest at the rear of the building to turn the knob to operate either the <br />UST or AST system or turn both system off. The system is set up to run one system at a time but neither <br />simultaneously together. <br />-When the knob was switched to UST, the East Diesel UST turbine was on and AST turbine was off. <br />-When the knob was switched to OFF, both the East Diesel UST and AST turbines system was off. <br />-When the knob was switched to AST, both the East Diesel UST and AST turbines system was on. <br />-Both UST and AST diesel was on, thus the above normal operating pressure reading on the line leak <br />detector tester. Knob switch system was verified by feeling the vibrations of the turbines. <br />-It is unclear if line leak detector is defective and needs replacement or if UST -AST hybrid system is not <br />allowing line leak detector to correctly operate. Knob switch system is not working correctly to isolate <br />between UST and AST system. <br />-First, immediately have a properly licensed, trained and certified contractor verify the operation of the <br />knob switch system to ensure the UST -AST system is set up to run one system at a time. <br />-All line leak detectors shall be capable of detecting a 3 -gallon per hour leak at 10 psi. If the line leak <br />detector is defective, have a properly licensed, trained, and certified contractor repair or replace the <br />failed leak detector (LG 113-x listed, if applicable) and obtain a permit from the EHD. If the failed leak <br />detector can't be replaced immediately, there is a possibility that the East diesel UST system may be red <br />tagged to prevent fuel inputs. <br />- Schedule a retest of the line leak detector with SJC EHD with a 48 hours test notification. Testing will <br />be billable at EHD's current hourly rate. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />Inspector: I Received By: Title: <br />=_HD 23-02 Rev 09/01/11 Page 3 UST INS ACTION REPORT <br />
